What types of home care booklets does MedForms® publish?
We publish custom booklets for Home Health, Hospice, Private Services, Home Medical Equipment (HME), Home Infusion and Pediatric agencies. We also offer Disease Process/Patient Education Books through Briggs Corporation.
What is included in a custom booklet?
A custom booklet combines your policies and procedures with state-specific regulations and accrediting body information (if applicable) into a professionally formatted booklet detailed to meet your agency’s needs. Clients also benefit from MedForms’® custom forms design and a wide selection of professionally formatted forms.
Can we customize the cover?
Booklet covers (front and back) are fully customizable with your agency’s logo, information and corresponding colors. When you submit your materials for development, please email us your logo in a vector file (these file types end in either “.eps” or “.ai”) to firstname.lastname@example.org. If your logo is in color, please send us the corresponding Pantone Matching System (PMS) colors. All covers are printed on high-quality cover stock in color ink at no additional cost.
What is the cost of a custom booklet?
Booklet cost is based on the number of pages in the booklet; quantity ordered, upcharges such as carbonless 2-part NCR forms, etc. The average cost per booklet is between $4.00 – $6.00. As the purpose of the booklet is to simplify the admission process and minimize costs, we will advise you during the drafting process where information may be streamlined and ways to keep the cost of the booklet to a minimum. An exact cost per booklet will be provided to you once the custom booklet is finalized.
A set-up fee will be associated with orders under 5,000 and due prior to development.
Do the booklets have set formatting?
All booklets are custom and can be designed to meet each individual agency’s requested specifications; however, we have thoughtfully formatted our master templates to be easily understood by both patient and clinician. With content that is written on a 5th – 6th grade reading level, we purposefully format booklets in large fonts, allowing for readability and comprehension. For agencies with corporate branding requirements (font type and size), we adjust formatting to meet marketing style guidelines.
What if we have changes/updates to make to the booklet?
MedForms® does not charge for revisions, a huge cost savings for our clients as changes to state, federal and accrediting organization rules/standards change regularly. MedForms® has a knowledgeable and efficient staff of editors who make all changes to existing booklets during the reorder process. When you reorder your booklets, our editorial staff will make any changes you request and also offer our suggested regulatory changes at that time, if applicable.

What if our agency is a small start-up?
Our “Startup/Small Company Package” applies to *home care agencies admitting 10 patients or less per month. The package cost is $825.00 and includes development of your custom booklet as well as your first 25 booklets to get started (shipping is included).
*We have a morotorium on Small/Start-up agencies in the states of Michigan and Texas at this time as well as Miami-Dade, FL and metro Chicago.
What is included in a custom booklet?
MedForms® begins the customization process by using our master template. Once we receive your agency specific information, our Development Team will work with you on incorporating your materials into an all-inclusive booklet. Our Development Team has extensive knowledge of clinical and administrative aspects of the home care industry and works closely with you during the customization process.
Regulatory, Record-Keeping, and Accreditations FAQs
Does MedForms provide regulatory updates?
Yes. While MedForms® does not provide consultant services, our Regulatory Review Team researches and confirms home care regulations on a routine basis, allowing MedForms® to offer our clients suggested changes as state, federal and accrediting organization rules/standards change. We also update the booklets based on surveyor trends as clients report feedback from their accrediting and/or state surveys. While the ultimate responsibility of booklet compliance remains with the agency, we provide this valuable service to our clients free of charge.
What if we use point-of-care software or electronic records?
If your agency uses point-of-care software or electronic records, please notify MedForms® and a license agreement authorizing electronic permission will be provided to you. The license agreement for electronic reproduction permits your software vendor to electronically reproduce MedForms’® copyrighted forms (contained in your custom Patient Orientation Booklet) for the sole purpose of implementing an electronic record system.
What if our agency is accredited or seeking accrediation?
We regularly review and update our master verbiage to incorporate the latest JCAHO, ACHC and CHAP accrediting body standards based on our interpretation of their application in the patient orientation booklets. If you are seeking accreditation, please specify the accrediting body during the development phase. We will review your booklet and incorporate changes as we feel necessary; however, as we are not a consultant, we do also ask that you review the standards and your booklet to ensure compliance.
Printing, Shipping and Languages FAQs
What if our agency has multiple locations?
We work with clients ranging in size from small, one-location companies to large national, multi-state, multi-location agencies. We have many different options for multi-location agencies including printing separate covers (with content of the booklet being shared so that larger quantity price breaks are still available) or by listing each location on the cover of the booklet and adding check boxes for the patient to reference. These needs will be addressed during the development process.
How many booklets should we order at at time?
We recommend ordering a 6-month supply of booklets at a time to ensure that you are receiving regular updates.
How long does it take to print and ship booklets?
From the time that we receive your written approval of any revisions, it takes approximately 2-3 weeks for booklets to arrive. For clients in Hawaii and Alaska, please allow an additional week of transit time.
Can we copy the booklet or have it printed somewhere else?
No. Duplicating copyrighted material is strictly prohibited. Our Patient Orientation Booklet is considered a unique and valuable asset of our company and is protected by U.S. Copyright laws under Title 17. MedForms® reserves all rights to printing, copying and using our booklets. To confidentially report copyright violations, please email email@example.com.
Do you offer Spanish language booklets?
Yes. We offer translation of your MedForms’® patient orientation booklet into Spanish for a $150 setup fee. Additional translation fees may apply. Average $4.00 – $6.00 booklet pricing and standard shipping costs will apply. Please contact us for more information.
