Q: In what format should I send my change requests?

A: Please send changes in writing. This eliminates confusion and helps make the editing process more efficient. You can read our post on how to reorder for more information.

Q: How long will it take to receive a proof when I reorder?

A: Our standard turnaround time for proofs is about 5-7 business days.

Q: Can my order be split-shipped to multiple locations?

A: Yes. When you are ordering, please send us the addresses and the quantities that need to be shipped to each location.

Q: How long will it take to receive the order once I approve my proof?

A: Once we receive approval from you, the timeframe for printing and shipping your patient admission booklets is 2-3 weeks. You will receive tracking information via email when your order is shipped.

Q: How can I pay my MedForms invoice?

A: When you receive an invoice from us, you can either mail us a check or call to pay by credit card. Please note that if you do pay with a credit card, there is a 2.75% processing fee, and we accept Visa, Mastercard and Discover (no American Express).

Q: How do I reorder online?

A: Go to medforms.com and click the “Reorder” tab. There is also a link to this page in all of our editors’ email signatures. Simply click the green reorder button (pictured below). This will take you directly to our order form that you can fill out and submit. Once you submit your reorder request, you’ll receive a confirmation email. If you do not receive this email, please call 1-800-843-9178.
