MedForms has updated the Hospice Election Statement and Hospice Election Addendum – titled, “Patient Notification of Hospice Non-Covered Items, Services, and Drugs” – in accordance with the new requirements outlined in the 2027 Hospice Wage Index Final Rule. These updates will be effective October 1, 2026.
The final rule includes the requirement to provide the Addendum to every patient, in writing, upon hospice election (that is, within the first 5 days of the effective date of the hospice election), rather than the previous requirement to provide upon patient request. Additionally, if there are any changes to the plan of care during the course of hospice care that impact the addendum determinations, the hospice must update and provide an Addendum, within 3 days. This results in modifications to the Addendum form itself, as well as changes to the Hospice Election Statement, informing patients when they will be receiving the addendum form.
